New icons on the web portal

You will see a two clipboard icons on the web portal. These buttons are not yet ready for public release but we thought we would let you know what they will do in advance.

The clipboard with the question mark is the button to produce your hospital’s Data Validation Report.

The clipboard with the exclamation mark should list all patients not meeting the minimum data set standard.

 

Visit outcome

There has been some confusion with two fields on the visit outcome page.

  • Discharge
  • Visit outcome (regarding further review)

We have changed the labels for these fields so that we hope their sense is clearer.

  • Discharge becomes “D/C from follow-up

Apologies for the ‘”D/C” abbreviation for discharge but there was limited space.  Tick here if at the end of the visit you are not planning to review the patient again (either because review is not clinically indicated (or) because the patient will be admitted to a critical care bed.

  • Visit outcome (regarding further review) becomes “Further review (Visit Outcome)

This should read better when followed by the text in the drop down list.  For example, it would now read “Further review (Visit Outcome) … Planned (No Rx limits or DNAR)”

Please let us know if you have any concerns.  The field definitions and choices remain completely unchanged.

Confusion over visit outcome fields

There are two fields designed to capture what happens to a patient at the end of the (SPOT)light visit.

The first is checkbox asking if you wish to discharge the patient.  Tick this if the patient will no longer be followed by critical care or outreach on the ward.  This may be because they are too well (and don’t need you) or too sick (and will be admitted to critical care).

The second is a drop down labelled “Visit outcome (regarding further review)”.

If you have ticked ‘discharge’ then you are telling us that there will be no further review but you now specify whether this is because the patient was accepted to a Level 2 or 3 bed (with the sickest patients) or because the patient was too well and further review was not indicated.  If you didn’t tick discharge then further review is ‘planned’ and all you are doing is indicating if treatment limits were in place or not.

We hope ‘patient died’ is self explanatory!

 

How to handle long patient lists on the web portal

Question: I have just been entering data on to the portal and our patient list is getting quite long. Do we need to discharge the patients who we know have died or gone home since they where 1st seen by critical care? I am just checking because I don’t want the list to get too long and it be a pain to do later.

Answer: We are working on a solution for this.  For now please don’t discharge patients just to keep the lists short.

Don’t forget the CMP number!

Please try and enter the CMP number even if the patient isn’t immediately admitted to critical care.

This is not difficult but it does mean you’ll need a system in place to check that when ever a patient from the ward is admitted to ICU you go to the web portal and enter the CMP number against their (SPOT)light record (assuming they have one).

 

A query about the (SPOT)light patient list …

Is there an order that the names are listed ?

Yes. You can sort the patient list out in terms of the headings of the columns. When you hover over a heading, i.e. patient name – two little arrows appear, if you click that, this sorts the patient list alphabetically by surname.

Teething problems

We have been working hard (almost round the clock) trying to get the web portal ready.  We are sorry to say that we have had some teething problems, and we hope you understand that we would prefer to iron these out before you start using the system in anger.

This does mean further delays for which we are really sorry – but please check back here regularly.  We will email those sites we know who are ready to start data collecting the moment the system is fully functional.

If you believe your site is ready to start then please contact us at spot@icnarc.org for a final checklist to complete.  We will then activate your site – which means you will be able to register for the web portal.
